Recommended Action
  1. Approve agreement with Yolo Community Care Continuum in the amount of $551,738.30 for the period of February 20, 2024 to March 31, 2026, for the provision of medical respite/recuperative care to clients experiencing homelessness and/or are at risk of homelessness and are experiencing a health or behavioral health need; and
     
  2. Delegate authority to the Director of the Yolo County Health and Human Services Agency or designee to: 1) exercise optional extensions; and 2) issue related option year notices. Please see Section III. of Att. A. Agreement.

Reason for Recommended Action/Background
Partnership HealthPlan of California (“PHC”) is a non-profit community-based healthcare organization that contracts with the State of California to provide Medi-Cal services under a County Organized Health System model. The federal government approved a Home and Community-Based Services Plan to allow for a variety of programs, including the Housing and Homelessness Incentive Program (HHIP). The goals of the HHIP program are to reduce and prevent homelessness and to ensure that Medi-Cal managed care plans develop the necessary capacity and partnerships to connect their members to needed housing services. On or about January 24, 2023, the County entered into an agreement with PHC to receive funding for a HHIP Program in Yolo County.

As part of the HHIP Program, Yolo Community Care Continuum (“YCCC”) will provide recuperative care and/or medical respite services to individuals who are experiencing homelessness and/or are at risk of homelessness and are experiencing a health or behavioral health need. The services include interim housing with a bed and meals at YCCC’s Haven House (“Haven House”). Haven House provides short-term residential care for individuals who no longer require hospitalization, but still need to heal from an injury or illness (including behavioral health conditions) and would be exacerbated by an unstable living environment. Approval of this Agreement will allow the County to provide these vital services.

No general funds are required by this action. These services will be funded by Housing and Homelessness Incentive Program (HHIP) funding allocation. The total amount of this action is $551,738.30 for the period of February 20, 2024 through March 31, 2026. The amount of $67,196.87 is included in the HHSA adopted budget for fiscal year 2023-24; the amount of $272,329.49 is included in the HHSA requested budget for fiscal year 2024-25. The related funding will be included in the requested budget process for future fiscal years.

In no event shall the term of the Agreement extend beyond June 30, 2027, nor shall the total contract maximum exceed the amount of FIVE HUNDRED FIFTY-ONE THOUSAND SEVEN HUNDRED THIRTY-EIGHT DOLLARS AND THIRTY CENTS ($551,738.30), unless otherwise agreed to in writing by the parties and in conformity with the then-current Yolo County Procurement Policy approved by the Yolo County Board of Supervisors.
