Recommended Action
  1. Approve agreement with Turning Point Community Programs in the amount of $1,374,888 to provide Housing and Supportive services for Yolo County Full Service Partnership clients for the period October 1, 2017 through June 30, 2020; and
     
  2. Delegate authority to the Director of Health and Human Services Agency and/or her/his designee to sign and execute any additional forms, documents and/or amendments as necessary relating to this agreement.

Reason for Recommended Action/Background
This program will provide Housing and Supportive services to individuals enrolled in Transition Age Youth, Adult, and Older Adult Full Service Partnership (FSP) Mental Health Care Teams within the Yolo County Health and Human Services Agency (HHSA).  The purpose of the program will be to assist individuals to live in the least restrictive community-based setting appropriate to their needs, while reducing and preventing homelessness, involvement in the criminal justice system, and psychiatric hospitalizations/lengths of stay.
 
The performance measures included in this agreement are as follows:
 
Turning Point HSS  
Program Purpose Maintain stable housing in the least restrictive community-based setting appropriate to the needs of adults, transition age youth, and older adults who are experiencing serious behavioral health conditions, thereby reducing homelessness, involvement in the criminal justice system, and acute psychiatric hospital utilization.
Program Information Provide housing and related supportive services to HHSA-referred MHSA clients.
 
PM1: How much did we do?
1.1 Number of unduplicated adults, transition age youth, and older adults housed.
1.2 Number of weekly independent living skills (ILS) groups provided to program participants.
1.3 Number of program participants who complete program satisfaction survey.
PM2: How well did we do it?
2.1 At least 75% of persons referred to Housing and Supportive Services will be appropriately housed.
2.2 At least 75% of ILS group participants will be able to identify 1-2 independent living skills.
2.3 At least 75% of satisfaction survey results will demonstrate participant satisfaction.

PM3: Is anyone better off?
3.1 At least 75% of persons housed through the program will maintain stable housing in the least restrictive community-based setting appropriate to their needs.
3.2 Reduction in homelessness among program participants.
3.3 Reduction in criminal justice system involvement among program participants.
3.4 Reduction in acute psychiatric hospital utilization among program participants.
     
 
On June 21, 2017, Yolo County Health and Human Services Agency (HHSA) issued a Request for Proposals (RFP) from qualified community based organizations to provide Housing and Supportive services.
 
HHSA received one proposal from this solicitation.  The proposal submitted by Turning Point Community Programs was evaluated and determined to meet all the required qualifications necessary to successfully provide these services.  The Competitive Bid Process below provides a summary of the RFP evaluation criteria.

No general funds are required for this agreement.  These services will be funded by MHSA funds for the period October 1, 2017 through June 30, 2020.  The amount of $398,296 is included in the HHSA proposed budget for FY17-18.  The related funding will be included in the requested budget process for future fiscal years.
