Recommended Action
  1. Approve second amendment to Agreement No. 14-166 with Sutter Health Sacramento Sierra Region (dba Sutter Center for Psychiatry) to increase contract amount by $44,650 for fiscal year 2016-17, for a new contract maximum amount of $1,971,150, to provide Acute Psychiatric Inpatient Hospital Services to Yolo County residents for the period July 1, 2014 through June 30, 2019; and
     
  2. Delegate authority to the Director of Health and Human Services Agency and/or her/his designee to sign and execute any additional forms, documents and/or amendments as necessary relating to this agreement.

Reason for Recommended Action/Background
California state law mandated that psychiatric inpatient hospital services be provided to eligible Medi-Cal and non-Medi-Cal mental health consumers. Woodland Memorial Hospital is the designated and responsible hospital to provide such services to Yolo County residents adults (age 18 or older) who meet State Welfare and Institutions Code criteria and/or who are a danger to self, others or are gravely disabled and require inpatient acute psychiatric services.
 
When no adult bed space is available at Woodland Memorial Hospital, or when a child or adolescent is in need of services, the patient must be treated by another qualified psychiatric hospital facility in the surrounding area.  Sutter Center for Psychiatry is such a facility and as such, provides appropriate services in those instances to meet our consumer’s needs.
 
For services provided to Yolo Medi-Cal beneficiaries whose age is 20 or younger, the Contractor bills Medi-Cal directly to receive funding from federal financial participation and the County’s mental health realignment funds.  The State reduces the amount used for this purpose from the monthly mental health realignment apportionment to the County.  Because the payment to the Contractor is not directly via the County, the related costs are not stated in the contract. 
 
This Amendment is to include services provided to Yolo Medi-Cal beneficiaries whose age is 21 or older.  The Contractor is not certified to bill Medi-Cal for these services.  Nonetheless, the County is responsible for serving all Yolo Medi-Cal beneficiaries.  The stated amount is to be used for funding the services related to this particular group. The added funding for FY16-17 is to align with actual expenses.  The following is the breakdown of funding for this agreement:

All services described above require Yolo County Health and Human Services Agency (HHSA) approval.  The contract details the approval process and method of payment.  HHSA monitors the use of the services regardless of the patient’s age.  The HHSA budget includes the estimated funding needed for all services.
 
The performance measures included in this agreement are as follows:
 
Program Purpose To provide acute psychiatric inpatient services to eligible beneficiaries of Yolo County. To assist clients to resolve acute psychiatric crises and return to pre-crisis level of functioning.
Program Description Sutter Center for Psychiatry is an inpatient psychiatric hospital that provides voluntary and involuntary hospital-based inpatient psychiatric services to children, youth, and adults 24-hours a day, 7-days a week.

County Counsel has approved this Amendment as to form.

Competitive Bid Process
On July 31, 2007, the Board of Supervisors granted an approval to the Health and Human Services Agency (formerly known as the Department of Alcohol, Drug and Mental Health) for certain exceptions to the contract procurement policy.  According to this approval, hospital providers are classified as “Any willing providers” and are exempt from the competitive bid process.

No county general funds are required by this action. 
 
For the age range of 21 or younger population, approximately $50,000 per fiscal year in mental health realignment funds will be offset by the state for services that the contractor will bill directly to the fiscal intermediary.  HHSA has included the offset for each fiscal year in its requested budget, and will budget for subsequent years accordingly. 
 
For the age range of 21 or older population, HHSA is preparing a budget resolution separately to include the increase of $44,650 for FY16-17.  HHSA will budget the funding accordingly for FY17-18 and FY18-19.
