Recommended Action

Reason for Recommended Action/Background
When an adult resident of Yolo County with serious mental illness and serious emotional disturbances requires care in a locked facility (other than a State hospital), HHSA and local hospitals look for available bed space. Currently, these clients receive care in regional hospitals at a cost to Yolo County of $665-950/day.
 
This contract allows HHSA to purchase four beds at a high quality, affordable, inpatient psychiatric health facility. The beds are available to the County for patient placement without wait times. Based on historic trends, we expect these beds to be consistently full. Even if they are occasionally empty, we still project an overall savings.
 
The services provided by North Valley Behavioral will be billable to Medi-Cal and are therefore a more cost-effective option for this important level of care than free-standing psychiatric hospitals whose services are not billable to Medi-Cal, and which would otherwise be used by Yolo County for much of the inpatient care.
 
The increased amount for this Second Amendment is needed to agree with actual expenditures.  The cost savings will come from another underutilized contract.
 
Approval of this Amendment will provide psychiatric health facility services at a substantially lower cost for patients requiring inpatient interventions to treat a psychiatric crisis or other acute mental health problems.

Program Purpose To assist clients to resolve acute psychiatric crises and return to pre-crisis level of functioning.
Program Information North Valley Behavioral Health is a Psychiatric Health Facility (PHF) that provides voluntary and inpatient psychiatric services to adults 24-hours a day, 7-days a week.

Competitive Bid Process
On July 31, 2007, the Board of Supervisors granted an approval to the Yolo County Health and Human Services Agency (formerly known as the Department of Alcohol, Drug, and Mental Health) for certain exceptions to the contract procurement policy.  According to this approval, 24-hour residential adult mental health services are classified as “Any Willing Provider” and are exempt from the competitive bid process.

No general funds are required by this action.  These services will be funded by Medi-Cal and Realignment funds.  The increase amount of $115,000 for FY17-18 is cost savings from another underutilized contract.  The related funding will be included in the requested budget process for future fiscal years.
