- Approve fourth amendment to Agreement No. 14-131 with Yolo Community Care Continuum (YCCC) for the Adult Residential Treatment (Farmhouse) Services, to extend the term through June 30, 2019; include funding for the extended term; adjust contract maximum to remove scope of services for the Access to Care for Homeless and the Indigent (ACHIP) effective July 1, 2017, for a new contract maximum amount of $3,010,734; and update contract language where appropriate to clarify and refine the terms, conditions and requirements of the Agreement accurately; and
- Approve sixth amendment to Agreement No. 14-64 with Yolo Community Care Continuum (YCCC) for the Crisis Residential (Safe Harbor) and After Hours Phone Triage Services, to extend the term through June 30, 2019; include funding for the extended term, for a new contract maximum amount of $3,867,730; and update contract language where appropriate to clarify and refine the terms, conditions and requirements of the Agreement accurately.

These two (2) amendments with YCCC extend the term through June 30, 2019; increase maximum compensation; and revise and add language where appropriate to clarify and refine the terms, conditions and requirements of these Agreements, specifically as follows:
1. Adult Residential Treatment (Farmhouse) Amendment #4 -
The YCCC Farmhouse facility and program provides long-term residential treatment services to seriously mentally ill adults. The goals of this program are to help clients increase their ability to live independently through skill building, job training and/or other educational programs to further their independence thereby reducing psychiatric hospital stays, Institute for Mental Disease placements, and homelessness.
Effective July 1, 2017, the ACHIP services will be removed from this agreement. The services provided through that contract are now being assumed under other programs and funding.
2. Crisis Residential (Safe Harbor) and After Hours Phone Triage Amendment #6 -
YCCC provides Crisis Residential services to Yolo County consumers experiencing acute or severe psychiatric episodes requiring temporary removal from their home environment, and who meet the mental health medical necessity criteria.
The scope of services for the Crisis Residential (Safe Harbor) services has been modified to include the Housing Placement Assistance with one part-time Housing Navigator to work exclusively with HHSA clients at Safe Harbor on appropriate housing referral and placement including discharge planning per the approved Yolo County Substance Abuse and Mental Health Services Administration (SAMHSA) grant program.
The After Hours Phone Triage services provides information about services needed to treat urgent conditions; how to access specialty mental health services; and how to use the beneficiary program resolution and Fair Hearing process. There are no changes to this scope of service.
Approval of these amendments will further the Health and Human Services Agency’s efforts to support community-based services and prevent higher cost of care from occurring more than necessary.

Competitive Bid Process:
1. Adult Residential Treatment (Farmhouse)
On April 7, 2014, Alcohol, Drug and Mental Health (ADMH), now known as Health and Human Services Agency (HHSA), issued a Request for Proposals (RFP) requesting proposals from qualified providers interested in providing ACHIP services including benefit assistance, crisis mental health treatment, substance use disorder treatment, and supportive housing services for the un-served and underserved populations of Yolo County who are experiencing symptoms of mental illness and are seeking access to supportive services.
HHSA received one (1) proposal from this solicitation. After evaluation of the proposal received, YCCC was selected to provide these services.
On June 24, 2014, the Yolo County Board of Supervisors approved the agreement with YCCC to provide these services.
2. Crisis Residential (Safe Harbor) and After Hours Phone Triage
On February 25, 2014, Alcohol, Drug and Mental Health (ADMH), now known as Health and Human Services Agency (HHSA), issued a Request for Proposals (RFP) requesting proposals from qualified providers interested in providing Crisis Residential Services and an RFP requesting proposals from qualified providers interested in providing After Hours Phone Triage Services.
For the Crisis Residential Services RFP, HHSA received two (2) proposals from this solicitation. After evaluation of the proposals received, YCCC was selected to provide these services.
For the After Hours Phone Triage RFP, HHSA received three (3) proposals from this solicitation. After evaluation of the proposals received, YCCC was selected to provide these services.
On April 29, 2014, the Yolo County Board of Supervisors approved the agreement with YCCC to provide these services.
HHSA plans to issue Request for Proposals (RFP) for these services for the 2018-19 fiscal year. |
