- Approve agreement with CommuniCare Health Centers, Inc. for the Healthy Living with Diabetes program services for the uninsured County Medical Services Program and Medi-Cal eligible adults who are at high risk for diabetes and its complications in the amount of $259,556 for the period April 1, 2017 through March 31, 2020; and
- Delegate authority to the Director of the Yolo County Health and Human Services Agency or her/his designee to sign and execute any additional forms, documents and/or Amendments relating to this Agreement.

| Reason for Recommended Action/Background |
On February 7, 2017, the Yolo County Board of Supervisors approved an agreement with the County Medical Services Program (CMSP) Governing Board for the County Wellness and Prevention Pilot Project grant funding.
The purpose of this grant funding is to implement the Healthy Living with Diabetes program to reduce the burden of disease and its complications. The Healthy Living with Diabetes program is focused on implementing a public health diabetes education approach to reach up to 500 potentially eligible CMSP members with diabetes. The project is comprised of four main activities:
- Outreach and Enrollment for CMSP/Medi-Cal
- Linkage and Referral to Community Services
- Diabetes Self-Management Classes
- Home Visits
On February 7, HHSA issued a Request for Proposals (RFP) for a community based comprehensive health services provider to implement the Healthy Living with Diabetes Program. HHSA received one proposal from this solicitation. The proposal submitted by CommuniCare Health Centers, Inc. was evaluated and determined to meet all the required qualifications necessary to successfully implement the Healthy Living with Diabetes Program. |

| No general funds are required for this Agreement. These services will be funded by CMSP grant funds in the amount of $259,556 for the period April 1, 2017 through March 31, 2020. The full contract amount will be included in future fiscal year budget submissions. For the current fiscal year, if a 25% prorated amount ($21,630) is expended then we estimate sufficient appropriations already exist. |
