Approve Community Services Block Grant (CSBG) Agreement No. 17F-2052 with the State of California, Department of Community Services Development in the initial amount of $77,799 for program year 2017 in order to receive grant funds from the State, put the County's CSBG Community Action Plan into effect and enter into CSBG award agreements with service providers; and adopt resolution authorizing submittal of the Grant Agreement to the Department of Community Services and Development. Approve Agreement with Yolo Wayfarer Center dba Fourth and Hope to provide services for the Extended Hope Project, which include: identification, assessment and triage; intensive case management and treatment; and housing navigation services to clients with co-occurring mental health and substance use disorders experiencing homelessness in Yolo County in the amount of $1,935,181 for the period January 11, 2017 through September 30, 2019; and adopt budget resolution in the amount of $799,776 to appropriate the Cooperative Agreement to Benefit Homeless Individuals grant award from the Substance Abuse and Mental Health Services Administration in the Health & Human Services Agency 2016-17 budget. (No general fund impact) (4/5 vote required) (Larsen)
