Approve revenue agreement with County Medical Services Program Governing Board for the County Wellness and Prevention Pilot Project in the amount of $300,000 for the period January 1, 2017 through June 30, 2020. (No general fund impact) (Larsen)

Recommended Action
  1. Approve revenue agreement with County Medical Services Program Governing Board for the County Wellness and Prevention Pilot Project in the amount of $300,000 for the period January 1, 2017 through June 30, 2020; and
     
  2. Delegate authority to the Director of the Yolo County Health and Human Services Agency or her/his designee to sign and execute any additional forms, documents and/or Amendments as necessary relating to this Agreement.

Reason for Recommended Action/Background
This is a revenue agreement to accept funds from the County Medical Services Program (CMSP) Governing Board for the purposes of the CMSP County Wellness and Prevention Pilot Project which the Health and Human Services Agency was successfully awarded.
 
Approval of this Agreement will allow the Health and Human Services Agency to accept this grant funding for the purposes of creating the Healthy Living with Diabetes program to reduce the burden of disease and its complications particularly in the uninsured and Medi-Cal population.
 
The Healthy Living with Diabetes program is focused on implementing a public health diabetes education approach to reach up to 500 potentially eligible CMSP members with diabetes. This project is comprised of four main activities:
  • Outreach and Enrollment for CMSP/Medi-Cal
  • Linkage and Referral to Community Services
  • Diabetes Self-Management Classes
  • Home Visits
The Health and Human Services Agency plans to release a Request for Proposals and subsequently award funds to a community based comprehensive health services provider to develop and implement the Healthy Living with Diabetes Program.

No general funds are required for this Agreement.  This Agreement is funded by CMSP grant funds in the amount of $300,000 for the period January 1, 2017 through June 30, 2020.  Sufficient appropriations are already included in the adopted FY1617 budget and if accepted this grant will be included in the budget planning process for future fiscal years.
