Approve agreement with Winters Healthcare Foundation, Inc. related to 2013-14 Intergovernmental Transfer funding in the amount of $265,055, for the period of December 15, 2015 through June 30, 2016, to purchase land for the purpose of building a new healthcare facility to support the provision of healthcare services for the residents of Winters and nearby rural communities.  (No general fund impact) (Planell)
Recommended Action
A. Approve Agreement with Winters Healthcare Foundation (WHF) related to Intergovernmental Transfer (IGT) funding to purchase land for the purpose of building a new healthcare facility to support the provision of healthcare services for the residents of Winters and nearby rural communities. IGT funding will be in the amount of $265,055 for the period of December 15, 2015 through June 30, 2016.
B. Delegate authority to the Director of the Yolo County Health and Human Services Agency or her/his designee to sign and execute any additional forms, documents and/or Amendments as necessary relating to these Agreements.

Reason for Recommended Action/Background
On May 19, 2015 the Yolo County Board of Supervisors approved the pursuit by the Health and Human Services Agency of an Intergovernmental Transfer for Fiscal Year 2013-14 with the State Department of Health Care Services to secure federal matching funds to be used to provide additional health services for Medi-Cal beneficiaries in Yolo County, as attached. In the process outside agencies submitted proposals for startup programs and related costs. WHF's proposal to purchase land was amongst those proposals approved by the Board of Supervisors.

WHF currently operates three very small facilities, a 2,000 square foot medical office, and 850 square foot dental office, and an off-site administrative office. All sites operate in leased facilities that are not designed to support the provision of healthcare services. The facilities are too small to accommodate patients and their families, staff and equipment, and leave no opportunity for expansion.

Unlike other rural communities that are shrinking, Winters continues to experience a growth in population. Between 2005 and 2015, the number of patients served by WHF tripled. The enactment of the Patient Accountability and Affordable Care Act led to significant increases and demand for services at WHF facilities.

During the 2014 five-year strategic planning sessions, a new co-located facility was identified as a crucial need. This project has been embraced as a top strategic initiative by the WHF Board of Directors. In order to accommodate future demand for services, WHF desperately needs a larger facility. A new larger facility will allow WHF to co-locate all services under one roof thereby improving coordination of patient care, enhancing staff unity, and reinforcing WHF operations and principles.

Currently, WHF is in negotiation with a private developer to purchase a two-acre parcel, as part of a larger six-acre parcel on Grant Avenue coming into Winters. The site is located across from the Yolo County Health and Human Services Center.

Approval of this Agreement will support the use of the Intergovernmental funding used to purchase land so that WHF may build a healthcare facility to provide optimal space and staffing levels and provide additional health services for Medi-Cal beneficiaries in Yolo County.

This Agreement is funded by IGT funds for the period December 15, 2015 through June 30, 2016 for the amount detailed below:

The Yolo County Health and Human Services Agency has worked closely with Partnership HealthPlan of California to secure this funding along with other County staff including County Counsel and the Department of Financial Services. HHSA has also worked closely with stakeholders on the use of the funds, including the Yolo County Health Council, Oral Health Workgroup, Communicare Health Centers, Winters Health Care Foundation and First 5 Commission.

There are no general funds required for this Agreement.  This Agreement will be funded by Intergovernmental Transfer (IGT) funds being held in reserve for this purchase.  Expenditures and revenues are included in the adopted fiscal year 2015-16 Health and Human Services Agency budget.
