Brief Title:    CommuniCare IGT Funding Agreements FY15-17

Subject
Approve agreements with CommuniCare Health Services, Inc.to provide integrated behavioral health expansion services in the amount of $236,000 to extend access to behavioral health services for children, adolescents and families; case management services within the community clinics to coordinate care between hospitals and primary care providers in the amount of $70,000; pediatric specialty dental care services in the amount of $301,000 to allow more integrated medical/dental care for children to improve access to services and provide treatment in a timely manner; and transportation assistance services in the amount of $3,995 to provide taxi and/or other travel vouchers to assist 47 CommuniCare Health Center clients.  These agreements are collectively related to the Intergovernmental Transfer funding used to provide additional health services for Medi-Cal beneficiaries in Yolo County in the amount of $610,995 for the period January 1, 2016 through December 31, 2016. Recommended Action
  1. Approve Agreement with CommuniCare Health Centers, Inc. for the Integrated Behavioral Health Expansion services in the amount of $236,000 to extend access to behavioral health services for children, adolescents and families for the period January 1, 2016 through December 31, 2016.
     
  2. Approve Agreement with CommuniCare Health Centers, Inc. to provide Case Management services within the community clinics to coordinate care between hospitals and primary care providers in the amount of $70,000 for the period January 1, 2016 through December 31, 2016.
     
  3. Approve Agreement with CommuniCare Health Centers, Inc. for the Pediatric Specialty Dental Care services in the amount of $301,000 to allow more integrated medical/dental care for children to improve access to services and provide treatment in a timely manner for the period January 1, 2016 through December 31, 2016.
     
  4. Approve Agreement with CommuniCare Health Centers, Inc, for the Transportation Assistance services in the amount of $3,995 to provide taxi and/or other travel vouchers to assist forty seven (47) CommuniCare Health Center clients for the period January 1, 2016 through December 2016.
     
  5. Delegate authority to the Director of the Yolo County Health and Human Services Agency or her/his designee to sign and execute any additional forms, documents and/or Amendments as necessary relating to these Agreements.

Reason for Recommended Action/Background
On May 19, 2015 the Yolo County Board of Supervisors approved Yolo County Health and Human Services Agency (formerly known as the Department of Health Services) to pursue an Intergovernmental Transfer for Fiscal Years 2013-14 with the State Department of Health Care Services to secure federal matching funds to be used to provide additional health services for Medi-Cal beneficiaries in Yolo County, as attached.

Approval of these Agreements will support the use of the Intergovernmental funding for additional health services for Medi-Cal beneficiaries in Yolo County as follows:

Integrated Behavioral Health Expansion
CommuniCare Health Centers, Inc. will expand their Integrated Behavioral Health program to extend access to behavioral health services for children, adolescents and families. The expanded behavioral health services will provide residents of Yolo County with services essential to support the patient population accessing mental health services. This expansion will leverage current facilities, clinic hours, and primary care provider staff in providing a continuum of integrated mental health and primary care services.

Case Management Services
CommuniCare Health Centers, Inc. will provide case management services within the community clinics to coordinate care between hospitals and primary care providers with a special focus on high users of the emergency departments. Case managers will assist in mitigating unnecessary emergency department visits, enhancing care management, improving patient communication and providing patient education.

Pediatric Specialty Dental Care
CommuniCare Health Centers, Inc. will provide one full-time pedodontist and two full-time Registered Dental Assistants to address dental needs in a timely manner, allow more integrated medical/dental care for those children with the pedodontist and pediatricians for consult and referral, and provide pediatric patients and their families more options/flexibility to be treated in the hospital setting or in the outpatient dental clinic setting.

Currently there is a limited number of pedodontists in Yolo County and surrounding counties who accept Medi-Cal patients. A full time pedodontist position will allow the possibility to expand the pediatric dental program to address the shortage of specialty pediatric dental care in the community and increase access.

Transportation Assistance
This project will provide taxi and/or other travel vouchers to forty seven (47) CommuniCare Health Center patients.

These Agreements are funded by Intergovernmental Transfer (IGT) funds for the period January 1, 2016 through December 31, 2016 for the amounts as detailed below:

County Counsel has approved Agreements as to form.

The Yolo County Health and Human Services Agency (HHSA) has worked closely with Partnership HealthPlan of California to secure this funding as well as with other County staff such as County Counsel and the Department of Financial Services. HHSA also worked closely with stakeholders on the use of the funds, including the Yolo County Health Council, Oral Health Workgroup, Communicare Health Centers, Winters Health Care Foundation and First 5 Commission.

There are no general funds required for these Agreements. The Agreements will be funded by Intergovernmental Transfer (IGT) funds being held in reserve for these services. Expenditures and revenues are included in the adopted fiscal year 2015-16 Health and Human Services Agency budget. The related funding will be included in the requested budget process for Health and Human Services Agency for fiscal year 2016-17.
