Reason for Recommended Action/Background
The County of Sacramento, along with leaders from ten Northern California counties, is in the process of exploring the feasibility of establishing a Regional Mental Health Adult Correctional Facility to more effectively address the mental health treatment needs of incarcerated individuals in county jail facilities.

Facility Need
A significant proportion of jail inmates have mental health conditions and/or co-morbid chronic health conditions.  These inmates require psychiatric assessment and ongoing services while incarcerated.  County jails are facing increased demands due to the volume of inmates with serious mental health conditions needing services, the types of services needed, and legal pressures to substantially modify current practices.  Jails require psychiatric expertise and personnel to best assess and treat inmates.  Effective treatment and discharge planning for individuals in custody help with appropriate discharge to alternative treatment in the community, facilitates successful transition for those being released from jail, and reduces recidivism for those who are released.  

In response, leaders from ten Northern California Counties (Amador, Contra Costa, El Dorado, Nevada, Placer, Sacramento, Solano, Sutter, Yolo and Yuba) have discussed identifying more cost-effective approaches for addressing mental health treatment needs in county jail facilities. On behalf of these counties, Sacramento County released an RFP for a consultant to evaluate the feasibility of establishing and operating a Regional Mental Health Correctional Facility to meet assessment, treatment, custodial, and all other needs for inmates with moderate to serious mental health conditions who may be in a pre- and/or post-disposition phase of the criminal court process. The proposal from CGL Companies, LLC was selected for the feasibility study among the two proposals received after a collaborative scoring and review process with the counties.

Feasibility Study
As detailed in their proposal, CGL would conduct a feasibility study for the Regional Mental Health Correctional Facility in two phases. The first phase, occurring over a 90-day period, would cost $603,895 and involve four tasks (see Attachment B for details on each task):
  • Assess current services & treatment populations.
  • Develop a treatment model for centralized delivery of mental health services.
  • Conduct a cost/benefit analysis of the regional service delivery model.
  • Document findings in a Final Report.

The Final Report would be shared with project stakeholders in each of the counties. The counties would then determine if the project concept is feasible and whether to advance to Phase 2 of the project. Phase 2 would address development of a facility proto-type, site selection, and assessment of alternative approaches such as remodeling an existing facility or development on a new “green field” site. The full cost of all phases, if the counties proceed beyond Phase 1, would be $1.15 million.

Cost
The total cost of Phase 1 is $603,895 and would be split up among the participating counties. At this point in time, the County of Sacramento recommends portioning out the cost based on county population. The table in Attachment C shows what each county’s share would be using that approach. However, the costs are contingent upon the continued participation of all ten counties in the study. With that in mind, the estimated cost to Yolo County for the Phase 1 feasibility study is $31,714.69.

If the Board confirms Yolo County’s partnership in the feasibility study, it is the recommendation of the County Administrator that the Board approve the budget resolution (Attachment A) to appropriate up to $35,000 in General Fund contingency to cover Yolo County’s share of cost.

The County Administrator’s Office, Sheriff’s Office, and Health and Human Services Agency recommend partnering on the feasibility study project.

Bids Received
Each bid was reviewed and scored by stakeholders from several of the participating counties which narrowed the proposals to CGL Corporation, LLC, and Health Management Associates.  Prior to awarding the contract, representatives from Sacramento County, Yolo County, El Dorado County, and Contra Costa County held in person interviews with representatives from CGL and Health Management Associates to make a final determination on which proposal would be selected.
