Reason for Recommended Action/Background |
On or about January 2, 2020, the Yolo County Health and Human Services Agency (HHSA) submitted a grant application request to the Yolo County Grants Oversight Committee for approval of a new funding opportunity. On or about January 22, 2020, the Yolo County Grants Oversight Committee approved the request. This new allocation award is the resulting agreement.
Approval of this agreement allows Yolo County to receive a new allocation award in the amount of $1,067,700. HHSA intends to use approximately half of these funds to procure and subcontract services from local non-profits to identify, enroll, and retain an estimated 7,900 Yolo County Medi-Cal recipients. This funding will allow HHSA to maintain and grow the Medi-Cal caseload from target populations in Yolo County, as well as provide enhanced services to Yolo County Medi-Cal recipients. The remainder of this funding may also be used to cover administrative overhead costs and to supplement the hiring or subcontracting of 1-2 full time Yolo County Navigator staff persons. These staff persons will assist high usage Medi-Cal recipients with accessing the appropriate levels of care for chronic disease treatment, substance use, or connecting to medical care homes, in lieu of emergency rooms. |
