Recommended Action |
- Approve and authorize Health and Human Services Agency to submit an application for the County Medical Services Program (CMSP) Health Systems Development Grant Program to receive funding from the CMSP Governing Board up-to the amount of $300,000 for the period May 1, 2019 through June 29, 2022; and
- Delegate authority to the Director of the Yolo County Health and Human Services Agency and/or her designee to sign and execute the grant application and any additional agreements, forms, documents and/or amendments as necessary relating to this grant application, provided there are no significant programmatic or financial changes.

Reason for Recommended Action/Background |
On January 2, 2018, the County Medical Services Program (CMSP) Governing Board released a Grant Request for Proposals (RFP) for the Health Systems Development Grant Program. This grant may be used to support county specific efforts to expand access to care for primary care, specialty care and/or behavioral health services, coordinate and/or integrate health and behavioral health care service systems, and/or strengthen the overall health care delivery system in the county across a range of health and behavioral health providers.
Yolo County is planning to apply for this one-time competitive funding of up to $300,000 to be used over a period of 36 months to strengthen the implementation of Syphilis control and prevention activities for the local population. The proposed project will focus on improving coordination of efforts across multiple stakeholders for addressing the rapidly growing Syphilis outbreak in Yolo County, including offering prioritized Syphilis screenings and treatment of pregnant women and people experiencing homelessness, and coordinating wraparound health and human services for that targeted population.
By approving the submission of this grant application, the Board of Supervisors is authorizing the Yolo County Health and Human Services Agency to submit a grant application, which if funded, may lead to a revenue agreement for the County of Yolo.
Considering the above mentioned information, staff hereby recommends that the Board of Supervisors approve and authorize the Yolo County Health and Human Services Agency to participate in the competitive process for the CMSP-Health Systems Development Grant. |

No general funds are required for the Agreement. If this grant is awarded, the Agreement shall be funded by CMSP grant funds up-to the amount of $300,000 for the period May 1, 2019 through June 29, 2022. After acceptance of the grant application, th e grant amount will be included in the budget planning process for future fiscal years.
