Recommended Action
Approve fifth amendment to Agreement No. 2014-0237 with Yolo Wayfarer Center, Inc. dba Fourth and Hope for community-based outreach services to individuals with mental illness experiencing homelessness, to extend the term through June 30, 2019, and to add funding for fiscal year 2018-19 by increasing the contract maximum by $10,019 for a new total contract maximum of $48,980.

Reason for Recommended Action/Background
Projects for Assistance in Transitions from Homelessness (PATH) is a program of the Substance Abuse and Mental Health Services Administration (SAMHSA), which funds services for people with a serious mental illness experiencing homelessness. PATH is a formula grant authorized by the Stewart B. McKinnley Homeless Assistance Amendments Act of 1990. PATH funds are provided directly to the State of California Department of Healthcare Services (DHCS), which in turn allocates the funds to local counties. Each year, the Yolo County Health and Human Services Agency submits an intended use plan to DCHS for these funds.
 
In fiscal year 2018-19, Fourth and Hope will use PATH funds to support staff outreach activities intended to engage homeless individuals during the nightly community meal at the Fourth and Hope shelter. Once engaged through these outreach services, participants will be provided services including case management, the development of individualized personal development plans, life-skills support, screening and assessment of service needs, referrals to primary health services, job training, and linkage to housing and education opportunities.

No general funds are required by this action. These services will be funded by PATH funds. The amount of $10,019 was included in the HHSA adopted budget for Fiscal Year 2018-19.

SAMSHA PATH funding requires non-federal contribution towards the cost of services in the amount of one dollar for each three dollars of PATH funds provided.  For fiscal year 2018-19, Yolo Wayfarer Center will provide a match totaling $3,674. (See Exhibit E of Att. A Agreement.)
