Reason for Recommended Action/Background
CalWORKs is a statewide assistance program that provides support to eligible families in California.  The program consists of a comprehensive statewide employment and training program.  The employment and training program, referred to as CalWORKs Employment Services (CWES) provides services to CalWORKs families to help remove barriers to employment.  Services may include mental health services in an effort to remove barriers to employment for families and to support family stabilization through the public assistance to work transition.  The County has a mandate from the California Department of Social Services to implement mental health services for CalWORKs eligible recipients.

Scope

To provide mental health (MH) services for California Work Opportunity and Responsibility services to Kids (CalWORKs) Employment Services (CWES) clients in Yolo County.

Program Purpose

CommuniCare Health Centers, will provide mental health assessments and referrals to any eligible Yolo County residents that fit the target population of this contract.  To assist in preventing the unnecessary separation of children from their families, improve the quality of care and services to children and their families, and ensure permanency for children by reuniting them with their parents, by adoption or by other permanent living arrangement by providing the above services to CalWORKS children who may also be in the Child Welfare system.

Program Information

CommuniCare Health Centers is an FQHC in Yolo County, California which specializes in substance abuse services, in addition to mental and physical health services, and dental services.  CommuniCare Health Centers offers a unique opportunity to Yolo County residents by providing all of these services within their agency allowing for a more comprehensive treatment approach to address client needs.  They are a certified/licensed outpatient and intensive outpatient substance use provider who employs several certified substance use counselors, as well as, several licensed or license-eligible mental health staff in order to provide comprehensive ongoing substance use and mental health treatment to their beneficiaries.

Bi-annually completed reports (unless information is captured in CalOMS in which case the 15-day rule applies, see Exhibit D):

a) Demographic Data, including but not limited to:

      i. Number of beneficiaries served, by age, gender, race/ethnicity, culture if known, or disability (e.g.  hearing impaired, seeing impaired, wheel-chair bound).

b) Service Data, including but not limited to:
  1. Type and quantity of services provided
  2. Progress, disposition and/or outcome of client treatment
  3. Date, time, and location of service provided
  4. Referrals/coordination for other services (including, but not limited to: mental health, and physical health) and referral disposition
  5. Referrals/coordination to/with other substance use disorder providers for continued care
  6. Total number of beneficiaries who completed their treatment episode
c) Participant Progress Reports, including but not limited to:
  1. Satisfactory or non-satisfactory progress with current treatment plan.
  2. Revisions to existing treatment plans.
  3. Start of new plan or end of plan.
Complete Monthly:
  1. Total number of new participants referred to vendor during month.
  2. Number of carry over participants and their activity.
  3. Total number of cases that were assessed and the assessment date during the month.
PM2: How well did we do it?
2.1 Bi-annually: Customer Satisfaction Surveys (POQI): Consumer satisfaction surveys shall be administered twice annually with results submitted to the County.
2.2 Quarterly: Initiation, Engagement, and Retention Rates
  1. Initiation Rates:
Number and percentage of beneficiaries who receive at least 1 service (individual, group, collateral, or case management) within 14 days of a diagnosis being established by your facility
  1. Engagement Rates:
Number and percentage of beneficiaries who receive two or more services (individual, group, collateral, or case management) within 30 days of a diagnosis being established by your facility
  1. Retention Rates:
 Number and percentage of beneficiaries who stayed for a minimum of two weeks that completed their entire treatment episode
2.3 Complete Monthly: No Show, Refusal of Service and Timeliness
  1. No Show Rates: Number and percentage of cases closed due to No Show (No show equals three (3) or more missed appointments) during the months.
  2. Refusal of Service Rates: Number and percentage of cases closed due to refusal of services prior to an assessment during the month.
  3. Timeliness Rates: Number and percentage of referrals assessed within fifteen (15) days of receipt.
PM3: Is anyone better off?
3.1 Bi-annually completed reports.  Outcome Data, including but not limited to:
  1. Number of beneficiaries who completed successfully that did not return for another treatment episode within 6 months
  2. Number of beneficiaries who successfully met 75% or more of their treatment goals
  3. Number of beneficiaries successfully meeting their treatment goals that are fully engaged in Welfare to Work services (working, in-school, subsidized employment or job-training) within three (3) months of completion of their treatment plan.
*HHSA will provide engagement statistics based on completion numbers from contractor.

County Counsel has approved this Agreement as to form.
Competitive Bid Process
Competitive Bid Process:
A Request for Proposals was released on May 21, 2018. Potential bidders were offered the opportunity to ask questions during a mandatory Bidders’ Conference on May 29, 2018, and the proposal remained open through July 2, 2018 (6 weeks).  This opportunity was shared with the behavioral health providers that currently contract with Yolo County Health and Human Services Agency (HHSA) during several monthly meetings of the Provider Stakeholder Work Group leading up to the release of the RFP. The opportunity was also posted publicly on the HHSA website, and shared with a range of partner email distribution lists, including the Health Council, Homeless and Poverty Action Coalition, Local Mental Health Board, Provider Stakeholder Work Group, and Substance Use Disorder Provider Network.  Yolo County received one proposal in response to the solicitation. The proposal from CommuniCare Health Centers, Inc. was evaluated and was determined to meet the required qualifications to provide these services.

No general funds are required by this action.  These services will be funded by State funds.  The amount of $211,801 is included in the HHSA adopted budget for FY18-19.

Option Years: 
The County may exercise its option to extend the term of the Agreement as follows:

In no event shall the term of the Agreement extend beyond June 30, 2021 nor shall the total contract maximum exceed the amount of $790,403, unless otherwise agreed to in writing by the parties and in conformity with the County of Yolo Procurement Policy, Minute Order No. 18-55, Item No. 32, approved by the Yolo County Board of Supervisors on March 20, 2018 (“County Procurement Policy”).
