Reason for Recommended Action/Background
Over the past several years, the Yolo County Health and Human Services Agency (HHSA) and its partner agencies, including CommuniCare Health Centers (CCHC) and C.O.R.E. Medical Clinic (CORE),have been working collaboratively to create positive outcomes for clients struggling with opioid use throughout Yolo County.Each of these partners has worked to accomplish both individual and joint efforts, that when combined, have significantly expanded the availability of opioid treatment for residents of Yolo County.
 
In 2016, CommuniCare Health Centers obtained a Health Resources and Services Administration grant to provide medication assisted treatment (MAT) services to clients who did not previously have access to this treatment option locally.  The purpose of this grant was targeted towards encouraging federally qualified health centers to implement MAT services as a piece of their primary care service delivery.    
 
In 2017, CCHC and CORE submitted and were awarded a joint proposal for a Hub and Spoke Grant through the Department of Healthcare Services (DHCS). This partnership utilizes a model that has been effective in Vermont at combatting the effects of the opioid epidemic. The Hub and Spoke model works by creating a centralized Hub (CORE acts as the Hub), which coordinates ongoing services with Spokes (CommuniCare acts as the spoke) to increase the availability of MAT services to populations that cannot receive this treatment through other means.
 
Most recently, on July 1st, 2018, Yolo County went live with expanded services under the Drug Medi-Cal Organized Delivery System (DMC-ODS), which allows for expanded Medi-Cal covered services. These services include the expansion of local Narcotic Treatment Programs (NTP)to provide three medications they may not have offered in the past.  Historically, Yolo County has not held a contract with an NTP provider, and NTP providers have only been required to provide a single medication. Under DMC-ODS, counties must contract with NTP providers and NTP providers must make four different medications available to clients.
