Reason for Recommended Action/Background
The After Hours Crisis services provided in this agreement are part of an effort to build the crisis continuum in Yolo County. These services will replace our previous mobile crisis system with a new model that will be able to respond to the hospitals as well as law enforcement after normal business hours, with county staff managing the daytime response and Heritage Oaks Hospital managing the after business hours response. This contract also includes the 24/7 Behavioral Health Access Line in its entirety. 
 
There are three service elements provided in this agreement, as detailed below:
 
1.         Triage Access Line Services
Provide triage services to calls coming into the 24/7 Behavioral Health (mental health and substance use services) Access Line for Yolo County Health and Human Services (HHSA) and assess and appropriately disposition service calls, urgent support requests and crisis calls to support the community. These services will be provided 24 hours a day, 7 days per week, 365 days per year.
 
2.         Law Enforcement After Hours Crisis Response Services
Provide after hours community based crisis response services to anyone in Yolo County who comes into contact with law enforcement during a psychiatric crisis to reduce the cost of residential placement and psychiatric hospitalization, reduce emergency room visits and incarceration for residents experiencing a psychiatric crisis, facilitate recovery, and avoid future crises.

3.         Hospital After Hours Crisis Response Services
Provide after hours community based crisis response services on behalf of Yolo County Health and Human Services (HHSA) to those who arrive at a hospital emergency room during a psychiatric crisis and require assessment and placement assistance to reduce the cost of residential placement and psychiatric hospitalization, reduce emergency room visits and incarceration for residents experiencing a psychiatric crisis, facilitate recovery, and avoid future crises. At this time this applies to all persons requiring evaluation at Sutter Davis and those with Yolo County Medi-Cal at Dignity Woodland Memorial.  
 
The performance measures included in this agreement are as follows:

Increase access to behavioral health services

Program Information

Receive calls coming into the 24/7 Behavioral Health (mental health and substance use services) Access Line for Yolo County Health and Human Services (HHSA), providing telephone triage to determine appropriate resolution of both request-for-service and crisis calls.

  2.         Law Enforcement After Hours Crisis Response Services
 

Program Purpose

Reduce emergency room visits, psychiatric hospitalizations and incarcerations of those experiencing mental health crises and who are in contact with law enforcement.

Program Information

To provide afterhours community based crisis response services to anyone in Yolo County who comes into contact with law enforcement during a psychiatric crisis to reduce the cost of residential placement and psychiatric hospitalization, reduce emergency room visits and incarceration for residents experiencing a psychiatric crisis, facilitate recovery, and avoid future crises.

County Counsel has approved this agreement as to form.

Competitive Bid Process
On December 20, 2017, Yolo County Health and Human Services Agency (HHSA) issued a request for proposals (RFP) from qualified community based organizations to provide After Hours Access and Crisis Services. The RFP was open for six weeks, and staff offered potential proposers an opportunity to ask questions during a Mandatory Bidders’ Conference held on January 3, 2018.  Proposers were offered the opportunity to submit proposals via the electronic BidSync system or by dropping off a paper proposal directly at HHSA.
 
HHSA received two proposals from this solicitation.  A Proposal Review Committee was formed that included three HHSA employees from different branches, and two members from outside entities including behavioral health, emergency services, law enforcement, and the community. The two proposals were evaluated, and because scoring results were very close, it was recommended that individual interviews be conducted in order to determine the best qualified proposer.
 
The proposal submitted by BHC Heritage Oaks Hospital was selected as the top choice by all members of the Proposal Review Committee, and was evaluated and determined to meet all the required qualifications necessary to successfully provide these services.

No general funds are required by this action.  The funding for these crisis services will include a combination of FFP Medi-Cal, Realignment, MHSA funding, IGT funding, and other state or local funds.
 
The amount of $809,637 is included in the HHSA adopted budgets for FY17-18 and FY18-19, as detailed below. The related funding will be included in the requested budget process for future fiscal years.

Option Years:  The County may exercise its option to extend the term of the Agreement, as follows:
  1. If the County exercises its first option year, the new term of the Agreement will be June 1, 2018 through June 30, 2021; adding a contract budget of $750,900 for FY 2020-21, providing a new total contract maximum of $2,311,437.

  2. If the County exercises the second option year, the new term of the Agreement will be June 1, 2018 through June 30, 2022, adding $750,900 for FY 2021-22 providing a new total contract maximum of $3,062,337.
     
  3. In no event shall the term of the Agreement extend beyond June 30, 2022 nor shall the total contract maximum exceed the amount of $3,062,337.00, unless otherwise agreed to in writing by the parties and in conformity with Minute Order No. 18-55, Item No. 32, approved by the Yolo County Board of Supervisors on March 20, 2018.
