Recommended Action
  1. Authorize Health & Human Services Agency to pursue an intergovernmental transfer with the California Department of Health Care Services for the 2017-18 cycle and any additional cycles added by the State through June 30, 2019; and
     
  2. Authorize the County Administrator to sign contracts and any other documents directly related to the pursuit of these funds.

Reason for Recommended Action/Background
Each year the Yolo County Health and Human Services Agency (HHSA) seeks to secure federal financial support for services delivered to the Medi-Cal population that have not previously received federal matching funds. The mechanism for securing these funds involves an intergovernmental transfer (IGT) process, through an agreement with the California Department of Health Care Services (DHCS). The IGT process requires that Yolo County transfer local funding to DHCS, who uses the funds to draw down additional federal matching funds. DHCS then transfers the original county funds along with the federal matching funds (minus an administrative fee) to the local Medi-Cal Health Plan (in Yolo County this is Partnership Health Plan of California). Partnership then awards these funds to their partnering entities, including Yolo County, to be spent on proposed projects that align with one of more of the focus areas identified by Partnership, including mental health, substance use, specialty care access, oral health and care coordination.
 
The amount of IGT funding available has not been finalized yet, but is currently estimated at up to $2.5 million in new funds for each funding cycle requested. Initial draft documents are indicating that a new “true-up” process may be added. This process would involve a review of the State Medicaid rate range approximately two years after the matching funds are transferred from the County to the State. This true-up process could result in either additional matching funds being due from the County, or previously transferred matching funds being refunded back.

HHSA is seeking approval from the Board of Supervisors to pursue intergovernmental transfer funds. The Department of Health Care Services requires prior Board approval but the turnaround time for signing IGT contracts and other directly related documents is very short, often less than one week. By providing approval of this action in advance of receiving contracts, the Health & Human Services Agency and County Administrator's Office will be able to work collaboratively to purse these funds within the short timeframes allowed by the State.

As discussed with the Board during the IGT presentation on February 20, 2018, HHSA is currently developing a policy and procedure that will provide direction on how the Agency allocates these funds in the future. The draft policy is currently under review, and will be shared with the Board in the next few months.
