Recommended Action
  1. Approve agreement with CommuniCare Health Centers, Inc. to provide the Navigation Center Program services in the amount of $1,856,004 for the period May 1, 2018 through June 30, 2020; and
     
  2. Delegate authority to the Director of Health and Human Services Agency and/or her/his designee to sign and execute any additional forms, documents and/or amendments as necessary relating to this agreement, provided there are no significant programmatic changes or increases in funding.

Reason for Recommended Action/Background
The Navigation Center program will provide triage, assessments and case management to individuals seeking mental health care in Yolo County. The purpose of the program will be to augment those services currently provided by Yolo County Health & Human Services Mental Health clinics, effectively expanding specialty mental health care through increasing engagement and effective navigation of individuals in need of mental health services who are not currently receiving services, or who are receiving services at an inadequate level to address their acute or chronic needs. In addition, this program will assist to reduce and prevent homelessness, involvement in the criminal justice system, psychiatric hospital admissions and length of stays through encouraging individuals to link with care appropriate to their needs.
 
The community-based drop in Navigation Centers will serve Yolo County Adults ages 18 and older. The Centers will provide a central location and staff to support three types of consumers:
 
  1. Those with mental health conditions who are not currently accessing services. 
     
  2. Those who have been recently released from jail, hospitals, or other institutions and are not currently accessing services. 
     
  3. Those consumers whose mental health may be currently well managed and stable, but who are experiencing instability due to underlying challenges with basic needs such as housing, food or employment.

Program Information This program will effectively expand specialty mental health care in Yolo County through increasing engagement and effective navigation of Yolo County residents in need of mental health care who are not currently receiving services, or who are receiving services at an inadequate level to address their acute or chronic needs. The program will provide mental health triage/screenings, specialty mental health assessments, substance use disorder assessments, case management, crisis intervention and practical supports.

County Counsel has approved this Agreement as to form.
Competitive Bid Process
On April 20, 2017, Yolo County Health and Human Services Agency (HHSA) issued a Request for Proposals (RFP) from qualified community based organizations to provide Navigation Center services.  HHSA received one proposal from this solicitation.
 
The Board of Supervisors recommended HHSA review the RFP process used in this solicitation. After review it was determined this RFP should be re-issued in an effort to ensure the RFP is fair, ethical and competitive, while also maintaining high quality services to Yolo County residents.
 
On July 24, 2017, HHSA re-issued an RFP desiring to attract qualified community based organizations to provide Navigation Center services. This second RFP included a revised scope of work; a mandatory Bidder’s Conference; a cover letter with a summary of services, requirements, and timelines; and an option to either submit a proposal through the BidSync process or hand deliver a proposal to HHSA. A notice was also sent out to approximately 60 potential bidders who had previously viewed the original RFP issued on April 20, 2017.
 
HHSA received one proposal from this second solicitation.  A Proposal Review Committee was formed that included two HHSA staff and one member from an outside entity. The proposal submitted by CommuniCare Health Centers, Inc. was evaluated and determined to meet all the required qualifications necessary to successfully provide these services.

The Navigation Center services will be funded by MHSA funds.  The amount of $167,182 is included in the HHSA adopted budget for FY17-18.  The related funding will be included in the requested budget process for future fiscal years.
