Recommended Action
  1. Adopt resolution authorizing the Yolo County Health and Human Services Agency to accept and administer funds from the Department of Health Care Services to initiate a Future Whole Person Care Pilot;
     
  2. Amend the 2019-20 authorized Equipment List approving the use of Whole Person Care Pilot funds to purchase a mobile medical/dental vehicle; and
     
  3. Adopt a budget resolution increasing the fiscal year 2019-20 budget by $592,155.86 for a mobile medical/dental vehicle in the medical services fund.

Reason for Recommended Action/Background
On October 18, 2019, the Department of Health Care Services (DHCS) released the application process and funding allocations for each County to initiate a Future Whole Person Care Pilot. The Yolo County Health and Human Services Agency (HHSA) submitted the application on December 2, 2019 and received confirmation of approval from DHCS on January 2, 2020. HHSA is scheduled to receive the allocation of $592,155.86 in March 2020.

A Whole Person Care Pilot program coordinates health, behavioral health and social services in a patient-centered manner with the goal of improved beneficiary health and well-being. Low-income populations are more likely to experience a multitude of health, behavioral health, and social needs, requiring them to seek care across multiple and fragmented systems. The need for a Whole Person Care approach arose due to unmet social, behavioral health, and health needs within vulnerable populations, and from the existing fragmentation of organization and financing of current health and human services systems.

This one-time allocation is designed to provide start-up funding to counties that are not currently participating in the state’s Whole Person Care Pilot program, to initiate Whole Person Care-like pilot capacity based on a county’s demonstration of interest and letters of support from local partners within the county.

HHSA intends to utilize the allocation to aid low-income populations and improve outcomes of vulnerable and high-risk populations by providing mobile medical services. These services will be provided via a mobile medical unit, equipped with medical and dental exam space and associated plumbing and electrical. The funds will cover the cost of vehicle purchase, maintenance, operation, and training required for staff to operate the vehicle.

By adopting the attached documents, the Board of Supervisors is formally authorizing the County to accept the one-time allocation from DHCS for the Whole Person Care Pilot allocation, authorizing staff to use the funds to purchase a mobile medical/dental vehicle, and amending the fiscal year 2019-20 Adopted Budget to include the Whole Person Care Pilot funds.
