Recommended Action
Approve a new agreement with Yolo Community Care Continuum in the amount of $55,342 for the period August 1, 2019 through June 30, 2020 for the provision of housing support services funded through the Mental Health Block Grant (MHBG).

Reason for Recommended Action/Background
The Yolo Community Care Continuum (YCCC) currently provides adult crisis residential mental health services and long-term residential treatment services to Yolo County consumers. This additional YCCC program of Housing Support Services will provide coordinated case management services to clients with a mental illness who reside in YCCC’s numerous supportive housing units throughout Yolo County. These services will include:
  1. Support of any Yolo County-referred clients seeking supportive housing options when a YCCC unit becomes available;
  2. Ensuring housing stability by providing ongoing support to already-housed clients;
  3. Ensuring timely access to available crisis diversion, assessment and support services, if and when needed, to prevent clients from making emergency room visits and avoid inpatient hospitalization;
  4. Assessment of clients’ independent living skills bi-annually;
  5. Provision of health education as well as independent living skills training to meet clients’ specific needs in an individualized way;
  6. Provision of a vocation and employment needs assessments for clients, along with assistance and linkage to available education and employment resources in Yolo County; and
  7. Provision of intensive support and follow-up care to client’s post-hospital discharge to ensure successful transition back into housing.

This agreement is being sought by a Sole Source, which has been approved by the Procurement Manager.

No general funds are required by this action. The total amount of this action is $55,342 for the period of August 1, 2019 through June 30, 2020. These services will be funded by SAMHSA Mental Health Block Grant (MHBG).  The amount of $55,342 is included in the HHSA adopted budget for 2019-20.
