On or about May 27, 2021 the County entered into an Agreement with Yolo Community Care Continuum to provide supportive housing units that accommodate a range of client needs and preferences that help secure and maintain permanent housing throughout the County and ensure adults age 18 years and older, with serious mental illness have a program in place that will provide independent living engagement and service linkage. The goal with this program is to support clients in securing and keeping their acquired housing, despite any crisis or ongoing psychiatric symptoms. The original agreement had an initial term of July 1, 2020 through June 30, 2021 and two optional extensions for fiscal years 2021-22 and 2022-23.
On or about October 21, 2021, the County exercised the optional extension for fiscal year 2021-22.
On or about November 15, 2022, the County exercised the optional extension for fiscal year 2022-23.
Approval of this amendment will allow the agreement to be extended through June 30, 2024, add funding for fiscal year 2023-24 to allow services to be continued. A new Request for Quotes (RFiQ) is set to be released later in FY 2023-24, for a new contract start date of July 1, 2024.
