Recommended Action
Approve second amendment to Agreement No. PO 3956 with Shores of Hope to extend the term of the agreement through June 30, 2024 and add funding in the amount of $33,479, for a new contract maximum of $149,937, for the period of July 1, 2020 through June 30, 2024, for the provision of homeless outreach and case management services funded through Mental Health Block Grant (MHBG).

Reason for Recommended Action/Background
On or about June 4, 2021, the County entered into an Agreement with Shores of Hope to provide homeless outreach and case management services for adults that have been diagnosed with mental health illness and have a history of homelessness or are currently as risk of homelessness. The goal with this program is to support clients in securing and keeping their acquired housing, despite any crisis or ongoing psychiatric symptoms. The original agreement had an initial term of July 1, 2020 through June 30, 2021 and two optional extensions for fiscal years 2021-22 and 2022-23.

On or about July 14, 2021, the County exercised the optional extension for fiscal year 2021-22.

On or about July 20, 2021, the Parties amended the agreement to update agreement language.

On or about December 22, 2022, the County exercised the optional extension for fiscal year 2022-23.

Approval of this amendment will allow the agreement to be extended through June 30, 2024, add funding for fiscal year 2023-24 to allow services to be continued. A new Request for Quotes (RFQ) is set to be released later in FY 2023-24, for a new contract start date of July 1, 2024.

County Counsel has approved this amendment as to form.
Competitive Bid Process/Vendor Performance
On June 25, 2020 the County issued a Request for Quotes (RFQ) to provide mental health services to adults with serious mental illness (SMI) and children with serious emotional disturbance (SED) under the Mental Health Block Grant (MHBG) funding to establish and expand the community-based systems of care. A portion of this funding was allocated to three service scopes. The RFQ included the three scopes; 1. Independent Living Engagement & Service Linkage, 2. City of Davis-Permanent Supportive Living Engagement & Service Linkage; and 3. City of West Sacramento Homeless Outreach and Case Management.

Shores of Hope responded to the RFQ under scope 3. Homeless Outreach and Case Management.
