Subject
Authorize the Department of Health Services (DHS) to accept the California Department of Public Health, Maternal, Child and Adolescent Health Division (CDPH/MCAH) grant award for $375,000 and First Five Yolo grant award for $50,000 to fund the California Home Visiting Program; authorize the Department of Health Services Director and/or her/his designee to sign any and all documents necessary to accept the grant awards; approve and authorize the Chair to sign Agreement with Yolo County Children’s Alliance in the amount of $385,000 to provide California Home Visiting Program Services for the period July 1, 2015 through June 30, 2016; and authorize the Department of Health Services Director and/or her/his designee to sign any additional documents necessary to execute the Agreement and any amendments to the Yolo County Children’s Alliance Agreement. Recommended Action
  1. Authorize the Department of Health Services (DHS) to accept the California Department of Public Health, Maternal, Child and Adolescent Health Division (CDPH/MCAH) grant award for $375,000 and First Five Yolo grant award for $50,000 to fund the California Home Visiting Program.
  2. Authorize the Department of Health Services Director and/or her/his designee to sign any and all documents necessary to accept the grant awards.
  3. Approve and authorize the Chair to sign Agreement with Yolo County Children’s Alliance in the amount of $385,000 to provide California Home Visiting Program Services for the period July 1, 2015 through June 30, 2016.

Reason for Recommended Action/Background
The Patient Protection and Affordable Care Act of 2010 established the Maternal, Infant and Early Childhood Home Visiting (MIECHV) Program to provide an opportunity for collaboration and partnership at the federal, state, and community levels to improve outcomes for families who reside in at-risk communities through evidence-based home visiting programs. The California Department of Public Health, Maternal, Child and Adolescent Health Division (CDPH/MCAH) has awarded County a twelve month grant for the implementation of Healthy Families America under the California Home Visiting Program (CHVP) in the amount of $375,000. Additionally, First Five Yolo has awarded County a grant of $50,000 to fund this program. County will use $385,000 to fund the Agreement with Yolo County Children’s Alliance (YCCA); and $40,000 to cover County administrative costs. Total County funds for YCCA is attached.  The California Home Visiting Program (CHVP) shall strive to develop collaborative community systems that protect and improve the health and developmental outcomes for California’s pregnant women, parents, and families. Approval of this Agreement will provide the parameters for implementing or expanding an existing Nurse-Family Partnership (NFP) or Healthy Families America (HFA) home visiting program in accordance with Federal MIECHV and State requirements to achieve positive outcomes for each of the following five goals: 1. Provide leadership and coordinate maternal and early childhood systems and supports to advance federal, state, and local efforts to improve the health and well-being of families in California 2. Cultivate strong communities 3. Promote maternal health and well-being 4. Improve infant and child health development 5. Strengthen family functioning.

No county general funds are required by this action. The total amount for this Agreement is $385,000 for one fiscal year: 2015-16 and will be included in the proposed budget for fiscal year 2015-16.
