Recommended Action
Approve and authorize the chair to sign Second Amendment to Personal Service Agreement with 7th Avenue Center, LLC to extend the one-on-one staff and individual services through June 30, 2013 and to increase maximum compensation by $37,250 for a new contract maximum of $122,350 for the period July 1, 2012 through June 30, 2013.

Reason for Recommended Action/Background
A Yolo County seriously mentally ill consumer is in need of additional one-on-one staff and individual services during their placement at 7th Avenue Center, LLC.

At the end of November 2012, the consumer was evaluated. This evaluation determined that the consumer needs additional one-on-one staff and individual services for a short term period. The Personal Service Agreement (PSA) was then amended to include funding for this added service accordingly. However, subsequent evaluations determined that this special service is still required, and as a result, additional funding for this PSA is needed.

Approval of this amendment will allow the county continued placement and to extend the period of time to receive specifically needed services for this seriously mentally ill adult, thereby reducing the risk of placement at a higher level of care facility.

On June 29, 2010 the Board of Supervisors approved and authorized Yolo County Alcohol, Drug and Mental Health (ADMH) to continue use of personal services agreements with residential and psychiatric facilities for children and adults based on individual consumer placement needs, rather than general facility agreements.

The Board of Supervisors further approved the delegation of authority to the director of ADMH to act as fiscal and programmatic administrative agent for approving personal services agreements with residential and psychiatric providers for children and adults. However, it was agreed that no individual personal services agreement approved by the director of ADMH would exceed the threshold of $100,000.

Accordingly, ADMH now seeks the Board of Supervisors’ approval for the above-mentioned PSAs as the necessary amendments increases the maximum compensation over the $100,000 threshold to ensure the services are provided to meet the need of these seriously mentally ill individuals

Competitive Bid Process:
The Board of Supervisor’s approved the delegation of authority to the Director of ADMH to act as fiscal and programmatic administrative agent for approving personal services agreements with residential and psychiatric providers for children and adults.

No county general funds are required by this action. The total cost of this Amendment is funded by Mental Health Realignment funds. The funding for the increased costs is made available from other underutilized contracts and is included in the Alcohol, Drug and Mental Health’s (ADMH) adopted budget for fiscal year 2012-13.
