Approve first amendment to agreement with Willow Glen Care Center to reduce contract amount for fiscal year 2012-13 by $83,640; extend term through June 30, 2014; add funding in the amount of $346,750 for fiscal year 2013-14 for a new contract maximum of $609,860; and revise and add contract languages in various parts and exhibits of the agreement to provide mental health services to Seriously Mentally Ill adults living in board and care for the period July 1, 2012 through June 30, 2014. Recommended Action
Approve and authorize the Chair to sign First Amendment to Agreement with Willow Glen Care Center to provide mental health services to Seriously Mentally Ill adults living in board and care for the period July 1, 2012 through June 30, 2014. Included in this amendment are the following items:
  1. Reduce fiscal year 2012-13 maximum compensation by $83,640 to reflect revised projected utilization;
     
  2. Extend the term of this Agreement through June 30, 2014;
     
  3. Add funding for fiscal year 2013-14 in the amount of $346,750;
     
  4. Revise and add contract languages in various parts and exhibits of the Agreement to reflect changes in the State Contracts; where appropriate, to clarify and refine the terms, conditions, and requirements accurately; and where appropriate, to include languages necessary for the extended term of the Agreement;
     
  5. Revise Exhibit E-1, Contract Budget for 2012-13 to reflect the reduced amount; and
     
  6. Include Exhibit E-2, Contract Budget for 2013-14.

Reason for Recommended Action/Background
Reducing this contract based on utilization for this fiscal year 2012-13, allows the department to redirect funds most effectively and efficiently, to ensure the provision of mental health services to clients in the least restrictive setting.

Willow Glen, an adult residential care facility, provides board and care services as well mental health services that support consumers in their efforts to restore, maintain and apply interpersonal and independent living skills and allows consumers to access community support systems.
Yolo County relies on facilities like Willow Glen to keep Seriously Mentally Ill (SMI) adults safe when all other treatment interventions have proven ineffective. Prior to seeking placement at such a residential care facility, these clients have undergone extensive interventions. Often these adults have experienced multiple acute care hospital stays. It should be noted that a decision to place clients at this level of care is a collaborative one. A case manager has been specifically assigned to work with this population. This case manager works closely with treatment staff and is well versed on client needs and placement options.
Approval of this amendment will maintain the medically necessary care provided to consumers currently in treatment at the facility and allow ADMH to continue to reduce the use of state hospitals, psychiatric hospitals, and Institutes for Mental Disease (IMD) by making this lower level of care available when consumers are ready to transit from higher levels of care. Again, approval will also ensure access to all services necessary for the consumers to live in the least restrictive setting possible.

Competitive Bid Process:
On July 31, 2007, the Board of Supervisors granted an approval to ADMH for certain exceptions to the contract procurement policy. According to this approval, Willow Glen Care Center is classified as “Any Willing Provider” and is exempt from the competitive bid process.

On May 21, 2013, the Board of Supervisors granted an approval to ADMH to continue to postpone to June 2014 the issuance of multiple competitive bids for existing services currently being provided by Community Based providers.

No county general funds are required by this action. The total cost for this amendment is funded by realignment funds and is included in the Alcohol, Drug and Mental Health’s (ADMH) budgets.

The reduction, $83,640, for fiscal year 2012-13 is based on revised projected utilization for ten clients placed at Willow Glen. This amount not used by Willow Glen, is redirected to other contracts for providing mental health services to clients.

The amount, $346,750, is included in the recommended budget for fiscal year 2013-14.

The net cost of this amendment is $263,110.
